LPN - Long Term Care - Pay Rate: $40.00+ per hour - Day/Evening Rotating Shifts - 26 week contract

Position Overview:
- Perform comprehensive nursing assessments and monitor residents for changes in condition.
- Develop, implement, and evaluate individualized care plans.
- Administer medications, treatments, and other nursing interventions safely and accurately.
- Monitor vital signs, symptoms, laboratory results, and overall health status.
- Respond promptly to emergencies and changes in residents’ conditions.
- Communicate significant findings and changes in condition to physicians and other healthcare providers.
- Supervise and collaborate with Licensed Practical Nurses (LPNs/LVNs), Certified Nursing Assistants (CNAs), and other care staff.
- Provide direction and support to nursing staff while maintaining appropriate delegation and supervision.
- Coordinate admissions, transfers, discharges, and follow-up care.
- Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
- Educate residents and families regarding medications, treatments, disease management, and ongoing care.
- Current and valid NY State RN license.
- Minimum of 1 year experience in long-term care nursing preferred.
- Basic Life Support (BLS) certification.
- Strong clinical skills and attention to detail.
- Effective communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Compassionate and patient-centered approach to care delivery.
