QUALIFICATIONS:

Clinical Master’s Degree in: Social Work, Psychology, other Behavioral Health or Human Services; Prefer at least two years’ experience in the provision of behavioral health treatment and/or rehabilitation for individuals with mental illness and/or autism and intellectual disabilities;  CBT or DBT training preferred; Driver's License and insured vehicle preferred; Ability to use electronic health record required. Basic skills in Excel and PowerPoint desired; Ability to relate positively to individuals with serious behavioral health issues who have a co-occurring diagnosis of autism or intellectual disability.

PHYSICAL DEMANDS:  Work is physically active and will include bending, lifting, stairs, and the like. Ability to perform crisis management interventions, including releases, once training is successfully completed. Ability to perform CPR once training is successfully completed. Ability to walk distances during psych rehab activities and other community outings with residents. 

JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by providing quality services and being an effective team member. This position has the responsibility to carry out treatment, rehabilitation and support functions, psychoeducation, and provide crisis intervention to residents, as well as, support and consultation to their families/supports. Recovery principles, evidence-based practices and trauma informed care is to be used in the provision of services. Provide service coordination for assigned residents including coordinating and monitoring the implementation of their treatment and rehabilitation plan by the residential team.

SPECIFIC DUTIES:

1.  Provide individual, group and/or family therapy at least weekly for all residents. 
2. Ensure that the delivery of clinical treatment is congruent with approved treatment models.
3. Ensure residents and families receive education regarding mental health, intellectual disabilities, substance use or other issues that arise
4. Be flexible in scheduling clinical treatment to meet resident and family needs
5. Provide crisis intervention and management as indicated
6. Maintain on-going communication and collaboration with family members, and with staff from agency resident is transitioning to, to include: scheduling, follow up and documentation of each therapeutic leave or program visit.
7. With the resident and treatment team, develop discharge plans and arrange resources defined in the discharge plan, including completing ODP Transition Plan if required.
8. Effectively operate the RTF-A in the absence of the Director
9. Monitor residential site to ensure health and safety of residents
10. Train residential staff in providing appropriate therapeutic responses to residents that are consistent with their treatment, behavior and rehabilitation plan/s.
