Registered Nurse RN

NOW HIRING: REGISTERED NURSES (RN)

• Conduct initial nursing assessments, admissions, and ongoing evaluations.
  
  
• Provide skilled nursing visits to home care and hospice clients.
  
  
• Develop, implement, and update individualized plans of care.
  
  
• Monitor client conditions, identify changes, and communicate concerns promptly.
  
  
• Administer medications, provide wound care, and perform other skilled nursing interventions as ordered.
  
  
• Complete supervisory visits and evaluate the quality of care provided.
  
  
• Educate clients and families regarding disease management, medications, safety, and end-of-life care.
  
  
• Coordinate services with physicians, nurse practitioners, hospice aides, social workers, chaplains, and other interdisciplinary team members.
  
  
• Participate in hospice Interdisciplinary Group (IDG) meetings as required.
  
  
• Complete accurate, timely clinical documentation in the electronic medical record (EMR).
  
  
• Provide compassionate end-of-life care, including death visits and pronouncements when authorized.
  
  
• Participate in scheduled on-call rotations, including evenings and weekends, as assigned.
  
  
• Maintain compliance with agency policies, Georgia nursing regulations, and applicable CMS requirements.
  
  

• Current, unrestricted Registered Nurse (RN) license in Georgia.
  
  
• Graduate of an accredited nursing program.
  
  
• Previous home health, home care, hospice, or related clinical experience preferred.
  
  
• Strong clinical assessment, critical-thinking, and documentation skills.
  
  
• Ability to work independently in a home-based care environment.
  
  
• Current CPR/BLS certification.
  
  
• Valid driver's license, reliable transportation, and current automobile insurance.
  
  
• Ability to travel to clients' homes throughout assigned service areas.
  
  
• Experience using electronic medical records preferred.
  
  
• Excellent interpersonal skills and a compassionate approach to client and family care.
  
  

• Full-Time: Regular scheduled nursing visits, admissions, supervisory visits, and participation in on-call rotations.
  
  
• Part-Time: Flexible scheduling based on agency needs and nurse availability.
  
  
• PRN (As Needed): Opportunities to provide nursing visits, admissions, on-call coverage, and additional support as needed.
