Schedule: Wednesdays & Fridays, 12:00 PM - 8:00 PM

Fresh Perspective Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one care for a client in the home setting. This role includes skilled nursing services and transportation assistance during schedule shifts.

Responsibilities: 

• Provide direct nursing care in accordance with physician orders and the care plan
• Administer medications and treatments
• Monitor and document changes in client condition
• Communicate with supervisor regarding client needs and concerns
• Complete accurate and timely documentation
• Provide transportation assistance as needed

Requirements

• Active LPN license in good standing
• Valid driver’s license, car insurance, and reliable transportation
• Ability to lift, transfer, and assist up to 40 lbs 
• Ability to uphold client confidentiality
• Ability to work independently in a home setting
• Excellent communication and interpersonal skills
