PAS Resource Specialist – Care Management

Schedule: Sunday–Thursday, 8:00 AM–4:30 PM
Setting: Inpatient Hospital

Position Summary

The PAS Resource Specialist supports the Care Management and Social Work teams with patient-facing and administrative responsibilities related to admission, continued stay, and hospital discharge.

This role requires frequent face-to-face interaction with patients and their families in hospital rooms. The Specialist will help explain and obtain required patient notices, support discharge processes, maintain resources, and ensure appropriate documentation is completed in the electronic medical record.

Education & Experience

Candidates should meet one of the following:

  • At least 1 year of experience in a medical office setting, including high-volume direct patient contact and appointment scheduling. Experience obtaining managed care authorizations may also be required.
  • OR 1.5 years of experience in a high-volume, direct public-contact role plus at least 6 months of medical office experience.

Additional experience/knowledge:

  • Inpatient hospital experience
  • Insurance knowledge
  • EPIC experience
  • Knowledge of medical terminology and the patient discharge process

Key Responsibilities

Admission & Continued Stay

  • Deliver, explain, and obtain patient signatures for required Observation notifications.
  • Deliver, explain, and obtain signatures for Condition Code 44 notices.
  • Scan completed documentation into patient charts using EPIC or obtain electronic signatures via tablet.
  • Ensure documentation meets applicable CMS requirements.

Hospital Discharge

  • Educate patients on Medicare Important Messages and the discharge appeal process.
  • Obtain signatures on Medicare Important Messages and ensure documentation is entered into the patient chart.
  • Assist with Detailed Notices of Discharge related to Medicare appeals.
  • Perform data entry for Utilization Review Committee case determinations.
  • Create discharge packets under the direction of Case Managers or Care Integration Specialists.
  • Schedule discharge transportation as directed.
  • Support Care Management teams with Medicare notification processes and next steps.

Administrative Support

  • Order and restock supplies and resources for Care Management programs.
  • Triage incoming faxes and appropriately scan, upload, or distribute documents.
  • Communicate skilled nursing facility and home health availability information.
  • Assist with monthly P-Card reconciliation.
  • Perform additional duties as assigned.

  • Excellent written and verbal communication skills

  • Proficiency with Excel data entry and basic Microsoft Word
  • Ability to multitask effectively in a fast-paced environment with frequent interruptions

  • Conflict-resolution skills
  • Comfortable communicating directly with patients and families
