Conduct screenings, evaluations, direct therapy, consultation, and family/caregiver instruction to support communication goals. Collaborate with caregivers and other healthcare providers to support each patients goals. Develop and implement individualized care plans  Document progress, reassess regularly, and update goals to promote meaningful communication outcomes.
