This is an exciting opportunity to provide high-quality, patient-centered care in both community and academic settings while helping bridge the gap between acute and ambulatory palliative services.

The selected physician will provide a mix of inpatient and outpatient consultative services, with a strong focus on building an innovative embedded ambulatory model in collaboration with key service lines. • Collaborate with hospitalists, specialists, APPs, social workers, chaplains, and hospice teams to optimize patient care.
  • Participate in teaching opportunities with residents and medical students.
  • Advance system-wide quality and strategic initiatives, including reducing length of stay and readmissions, improving mortality, and supporting transitions to hospice when appropriate.
  • Contribute to program development, interdisciplinary education, and continuous quality improvement.
  • Foster a culture of communication, compassion, and teamwork.
