Position Summary
Palm Valley is seeking a Nursing & Intake Coordinator to support safe, compassionate care in its residential behavioral health program. This hybrid role combines licensed vocational nursing duties, medication-related services, and coordination of resident admissions and intake workflows. The coordinator partners with clinical, medical, and operations teams to promote timely admissions, accurate documentation, continuity of care, and a positive resident experience. All clinical duties are performed within the California LVN scope of practice, applicable orders, facility policies, and required supervision or direction.

•   Observe clients self-administer medications authorized by California law, valid orders, training, and facility policy; maintain accurate medication observation records.
•   Assist with medication reconciliation, pharmacy coordination, medication inventory, storage, controlled-substance procedures where authorized, and timely refill requests.
•   Collect and document vital signs and other assigned clinical observations; escalate urgent symptoms and follow emergency response protocols.
•   Coordinate with prescribers, nursing leadership, and clinical staff on orders, follow-up care, and resident education within LVN scope.
•   Help with obtaining labs, creating lab orders, specimen handling, and coordinating blood draws for lab work.
Key Responsibilities — Admissions & Intake
•   Coordinate admission scheduling, referral communication, pre-admission information gathering, and readiness of required intake documents.
•   Help welcome incoming residents, review intake checklists, obtain and document information within assigned responsibilities, and identify items requiring licensed-provider review.
•   Complete assigned nursing intake data collection and contribute to assessments and care planning under appropriate direction; do not independently perform functions reserved for an RN or other authorized practitioner.
•   Coordinate with admissions, clinical, utilization review, and operations teams to support efficient transitions into care.
•   Ensure medication lists, orders, allergies, medical history, and other critical information are routed to the appropriate clinicians for review.
•   Complete clinical intake including Bio Psycho Social assessment, CSSRS, initial treatment plan, and any other intake documentation necessary.
Documentation, Safety & Collaboration
•   Maintain timely, accurate records in the electronic health record and applicable admission and medication systems.
•   Follow infection prevention, confidentiality, HIPAA, incident reporting, and resident-safety protocols.
•   Participate in handoffs, interdisciplinary meetings, quality-improvement activities, and training.
•   Identify workflow gaps and recommend improvements to intake coordination and nursing support.
