Job DetailsLevel: ExperiencedJob Location: Rochester, NY 14604Position Type: Full TimeEducation Level: 2 Year DegreeSalary Range: $18.00 - $19.50 HourlyJob Shift: DayJob Category: Health Care / CounselingJob Summary: The Care Manager is responsible for providing comprehensive care management and coordination for individuals with co-occurring chronic health and behavioral health conditions. This role involves integrating medical, behavioral health, and social services to ensure individuals receive the necessary care and support. Candidates must have their own reliable vehicle for this position. Key Responsibilities: Care Management: Conduct comprehensive assessments, outreach, and engagement activities while linking and coordinating services using evidence-based practices. Care Planning: Develop and manage client-centered care plans to integrate chemical dependency, mental health, and medical services. Coordination of Care: Collaborate with Finger Lakes Community Health Providers, FLACRA clinic providers, and other identified service providers for ongoing care coordination. Assessment & Linkages: Evaluate psychosocial supports and connect individuals with housing, employment, transportation, medical, dental, mental health care, and community resources. Communication: Utilize effective communication strategies to coordinate care among peers, medical staff, addiction and mental health providers, and community agencies using electronic devices, Telehealth, and other interactive technologies. Outcomes Monitoring: Measure, monitor, and improve quality outcomes, focusing on clinical indicators, patient satisfaction, and adherence to care plans. On-Call Duties: Participate in on-call rotation for nights and weekends as assigned. Skills & Experience: Strong ability to communicate effectively and work cooperatively with culturally diverse individuals, staff, and community service providers. Knowledge of local behavioral health services and substance abuse agencies. Ability to multitask in a fast-paced environment, with strong problem-solving, time management, and organizational skills. Ability to remain calm in a crisis while providing crisis intervention services. Proficient in typing (minimum 30 WPM). Proficient in Internet navigation, Microsoft Office, Outlook, Word, and Excel. Familiarity with the use of electronic health records (EHR). QualificationsMust-Have Qualifications & Skills: Associate’s Degree in Health or Human Services. Valid NYS Driver’s License. At least one year of experience working with recovery supports, community resources, housing, employment, and other professional and non-professional services. Experience in a behavioral health care setting. Excellent written and verbal communication skills. Ability to multitask and demonstrate composure in challenging situations. Nice-to-Have Qualifications: Bachelor’s Degree or higher in Health or Human Services. 2+ years of experience in Behavioral Health. Experience in Crisis Management.
