This role is crucial in helping patients regain mobility, improve physical function, and achieve their rehabilitation goals. The therapist will work collaboratively with a multidisciplinary team to ensure comprehensive care and optimal patient outcomes. Key Responsibilities: - Conduct thorough assessments of patients' physical conditions and develop individualized treatment plans. - Implement therapeutic exercises and interventions to improve patients' mobility, strength, and coordination. - Educate patients and their families on exercises and activities to be performed at home to enhance recovery. - Monitor and document patients' progress, adjusting treatment plans as necessary to achieve desired outcomes. - Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective patient care. - Maintain accurate and timely records of patient visits, treatments, and progress notes. - Adhere to all professional, ethical, and regulatory standards in the delivery of physical therapy services. - Participate in ongoing professional development to stay current with advancements in physical therapy practices.QualificationsRequired Education: - Doctorate in Physical Therapy from an accredited institution. - Must possess an active license in the State of Michigan in good standing. Required Experience: - Minimum of 1 year of experience as a licensed Physical Therapist, in a home health setting. - Proven track record of providing high-quality physical therapy services on a per-visit basis. Required Skills and Abilities: - Strong knowledge of physical therapy principles, techniques, and equipment. - Ability to assess patient needs and develop effective treatment plans. - Excellent communication skills for interacting with patients, families, and other healthcare professionals. - Strong organizational skills to manage multiple patient visits efficiently. - Ability to work independently and make clinical decisions in a home or community setting. - Proficiency in maintaining accurate patient records and documentation. - Demonstrated ability to adapt therapy techniques to meet the unique needs of elderly patients. - Experience with Wellsky/Kinnser and Forcura preferred  Work Days/ Hours: Contingent Position  Any day Monday - Friday, set own hours Paid travel time, paid mileage and pay per visit rates 3-4 admissions/week  On-Call Rotation: Yes 1 summer holiday, 1 winter holiday and 1 weekend a month
