Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.    ESSENTIAL DUTIES AND RESPONSIBILITIES Provides assessment, evaluation, plan development, collateral, crisis intervention, and intensive case management services   Facilitates the High Fidelity Wraparound Team development and process, ensuing smooth transition through the four phases of wraparound and emphasizing inclusion of natural supports  Develops, coordinates, and ensures the implementation of the client plan and wraparound plan of care  Monitors data on treatment progress and use this data to modify the client plan/wraparound plan of care as needed  Locates, coordinates and develops linguistically and culturally diverse informal and formal resources and networks for families in the communities where they reside  Oversees the development of effective crisis plans, utilizing natural support and, where needed, formal resources, and provide crisis intervention services to families 24 hours a day on on-call  Collaborates with CFWB Social Workers, and Probation Officers, and outside resources including schools, psychiatrists, community outreach programs, family advocacy programs, and others as appropriate   Participates in high fidelity and quality improvement efforts, including wrap coaching, ongoing training, fidelity measures, and supervisions   Sustains the team commitment to High Fidelity Wraparound values and approach  Coordinates efforts with Parent and Youth Partners and other appropriate staff in providing supports and services  Maintains accurate, complete, and timely case records per Medi-Cal requirements and SDCC policies   Required to work evenings as assigned to meet the needs of families   Fulfills training requirements including SmartCare, cultural competency, HIPAA, and all applicable EBP training  Maintains current licensure and or intern registration All other duties as assigned    LANGUAGE SKILLS Ability to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groups  REASONING ABILITY  Ability to read, analyze, and interpret medical records, journals, reports, and legal documents; Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community; Ability to effectively present information to management and public groups  PHYSICAL DEMANDS    The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. QualificationsMINIMUM REQUIREMENTS: Valid California Driver’s License and Reliable Transportation (must be willing to utilize personal vehicle for work related responsibilities) Master’s Degree from an Accredited University Registered Associate or in process with the BBS in Social Work, Psychology, Professional Clinical Counseling, or Marriage and Family Therapy and License Eligible in the State of California Must be highly organized, with excellent written and verbal communication skills Experience working with high risk youth Ability to work cohesively on a team PREFERRED QUALIFICATIONS: Proficiency with Cerner (online medical record system) Leadership, public speaking, or group therapy experience  Experience working with CFWB and Probation youth Experience working in a high fidelity wraparound program
