Role SummaryĀ 
The Per Diem Patient Care Partner (Float Pool) works under the direction of a RegisteredĀ 
Nurse (RN) or Licensed Practical Nurse (LPN) to provide direct patient care across multipleĀ 
patient care units. This role assists with activities of daily living, monitors patientĀ 
conditions, performs clinical support tasks, and promotes a safe, compassionateĀ 
environment for patients and families.Ā

ResponsibilitiesĀ 
Participates in the delivery of patient care for population groups under the direction ofĀ 
an RN/LPN including but not limited to the following:Ā 
• Lifting, turning, and positioning patients utilizing Safe Patient Handling equipment (>Ā 
35 pounds), as appropriate. Ā 
• Observing confused and difficult patients. Ā 
• AM/PM Hygiene care. Ā 
• Toileting, shaving, washing, brushing hair, dental and mouth care. Ā 
• Feeding. Ā 
• Assisting with range of motion exercises. Ā 
• Mobilizing patients ‐ transfers and ambulation, utilizing Safe Patient HandlingĀ 
equipment (> 35 pounds), as appropriate and following the Mobility protocol. Ā 
• Exercise protocols. Ā 
• Discontinuing foley catheter. Ā 
• Simple dressing changes. Ā 
• Incentive spirometry supervision. Ā 
• Surgical preps. Ā 
• Postmortem care. Ā 
• Administers cleansing enemas. Ā 
• Removal of peripheral IV catheters. Ā 
• Sits with confused/disoriented patients or those requiring 1:1 observation forĀ 
safety/suicide purposes, as assigned. Ā 
• Performs and records accurately: Ā 
o Temperature, pulse, respirations, blood pressure, heights and weights. Ā 
o I & O. Ā 
o Records bowel movements. Ā 
o ADLs and activities. Ā 
• Performs and records the following specimen collection: Ā 
o Obtains urine, stool, and sputum specimens for patients; instructs patientsĀ 
in proper specimen collection technique. Ā 
• Completes the following support activities: Ā 
o Completes EKGs. Ā 
o Transports patients as needed. Ā 
o Serves, sets up and retrieves trays. Ā 
o Distributes water pitchers as appropriate. Ā 
o Orders and distributes nourishment. Ā 
o Transports equipment. Ā 
o Transports blood products to and from the patient care area. Ā 
• Participates in patient safety/patient satisfaction. Ā 
• Answers call bells. Ā 
• Participates in patient rounding. Ā 
• Reports any signs of abuse to the nursing staff. Ā 
• Recognizes, troubleshoots and initiates corrective action needed on equipment. Ā 
• Maintains neat and tidy environment (empties laundry, delivers equipment, keepsĀ 
patient rooms clean and safe). Ā 
• Inventories and assures disposition of patients' belongings when admitted,Ā 
transferred, and/or discharged. Ā 
• Assures proper storage of equipment. Ā 
• Recognizes emergency situations and initiates plan of action. Ā 
• Notifies RN/LPN of any changes seen in patient's condition. Ā 
• Complies with policies and procedures of the hospital/nursing department. Ā 
• Supports the philosophy of the hospital and department of nursing. Ā 
• Maintains CPR certification and practice. Ā 
• Demonstrates cost-effective patient care by demonstrating proper use and care ofĀ 
equipment, appropriate and prudent use of supplies, accurate charging of supplies;Ā 
performing other division-specific tasks, and appropriate utilization of availableĀ 
resources. Ā 
• Participates in performance improvement activities to improve service and care.Ā 
Demonstrates strong communication and organizational skills. Ā 
• Ability to communicate using telephones, computer systems. Ā 
• Answers telephone promptly and politely, identifying self, title, and department.Ā 
Receives and sends messages in an accurate and timely fashion. Ā 
• Communicates with the patients, family, and members of the healthcare team in aĀ 
concise, tactful and considerate manner. Must represent the hospital in aĀ 
professional courteous manner, while being sensitive to how others perceive bothĀ 
verbal and non-verbal communications. Ā 
Other Duties:Ā 
• Assists in the orientation of new personnel and serves as a role model to otherĀ 
employees. Ā 
• Demonstrates willingness to accept non-routine work assignments as appropriate. Ā 
• Encouraged to participate in community activities. Ā 
• Attends and participates in unit council (70% attendance). Ā

QualificationsĀ 
EducationĀ 
• High School Diploma or GED required. Ā 
Ā 

ExperienceĀ 
Must meet one of the following qualifications:Ā 
• Successfully completed a Certified Nursing Assistant (CNA) training course; orĀ 
• Minimum of one (1) year of experience in a Nurse Aide or equivalent patient careĀ 
role; orĀ 
• Currently enrolled in a licensed nursing education program and have successfullyĀ 
completed a Fundamentals of Nursing course. Ā

• Provides direct patient care to patients of all ages and interacts with familyĀ

• Float assignments are based on organizational staffing and patient care needs. Ā 
• Current CPR certification required or obtained in accordance with organizationalĀ 
policy. Ā 
• Frequent standing, walking, bending, reaching, pushing, pulling, and liftingĀ 
throughout the workday. Ā 
• Ability to lift and carry up to 50 pounds and assist with moving patients over 100Ā 
pounds using Safe Patient Handling equipment. Ā 
• Regular exposure to blood, body fluids, communicable diseases, and otherĀ 
healthcare-related hazards. Ā 
• Must adhere to all hospital policies, infection prevention standards, patient safetyĀ 
guidelines, and quality improvement initiatives. Ā 
• Participation in orientation, unit council activities, ongoing education, andĀ 
professional development is encouraged. Ā
