Healthcare United is seeking Board Eligible or Board Certified Internal Medicine or Family Medicine physicians for outpatient primary care opportunities in Southbury, Naugatuck, Shelton, and Oxford, Connecticut.

There are multiple openings available for physicians seeking a full-time outpatient primary care opportunity in a supportive employed practice setting.

The incoming physicians will provide comprehensive primary care services, including wellness visits, preventive care, chronic disease management, acute visits, treatment planning, and care coordination.

Responsibilities

  • Provide outpatient internal medicine or family medicine care
  • Evaluate and manage adult or family medicine patients
  • Conduct wellness visits, sick visits, preventive care, and chronic disease management
  • Develop individualized treatment plans
  • Coordinate care with specialists, APPs, nurses, and clinical staff
  • Maintain accurate EMR documentation
  • Support patient access and continuity of care
  • Build long-term patient relationships within the community


Requirements

  • Board Eligible or Board Certified in Internal Medicine or Family Medicine
  • Active or eligible Connecticut medical license
  • Outpatient primary care experience preferred
  • Strong clinical assessment and communication skills
  • Interest in community-based outpatient medicine
