Licensed Practical Nurse

Concordia is seeking compassionate and dedicated Licensed Practical Nurse to provide high-quality nursing care and support to our residents. This role is responsible for administering medications, coordinating resident care, supervising nursing assistants, and ensuring residents receive exceptional care in a supportive environment.

Current openings: We are currently hiring for a Full Time Licensed Practical Nurse.

• Third Shift Hours: 10:30pm- 6:30am

*Pay is based on experience and shift, $33.75-$37.25*

*Shift Differentials include: Third shift, $2.00, and the weekend $3.00. These differentials are stackable!

Responsibilities for our Licensed Practical Nurse:

• Administer medications in accordance with physician orders, policies, and regulations
• Complete accurate and timely resident assessments and documentation
• Monitor resident conditions and communicate significant changes to physicians and families
• Carry out physician orders, referrals, and treatment plans
• Provide treatments and nursing procedures as needed
• Supervise and support Nurse Aides by monitoring care quality, documentation, staffing coverage, and daily assignments
• Participate in shift report communication to ensure continuity of care
• Assist with direct resident care as needed

Qualifications for our Licensed Practical Nurse:

• Current Pennsylvania LPN license required; Or multi-state license
• Compassionate, caring, and resident-focused approach
