Position Summary

We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered therapy services on a PRN basis. This role focuses on helping residents improve mobility, strength, and independence while working as part of an interdisciplinary care team.

Key Responsibilities

  • Evaluate residents and develop individualized treatment plans
  • Provide skilled therapy services per physician orders and care plans
  • Document evaluations, treatments, and progress notes in compliance with regulations
  • Communicate effectively with physicians, nursing staff, and care team members
  • Supervise PTAs, aides, and students in accordance with state practice acts
  • Participate in care planning and interdisciplinary meetings
  • Educate residents, families, and caregivers on therapy goals and discharge plans
  • Conduct staff training and in-service education
  • Maintain compliance with all state, federal, and payer guidelines

Qualifications

  • Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
  • Active South Carolina Physical Therapist license (in good standing)
  • Experience in skilled nursing or rehabilitation setting preferred
  • Familiarity with PointClickCare and Casamba preferred
  • Strong clinical, documentation, and communication skills
