Job DetailsJob Location: Willapa Harbor Hospital - South Bend, WA 98586Position Type: Per DiemSalary Range: $73.05 - $77.24 HourlyJob Shift: DayPAY:  $73.05 - $77.24 /hr BOE, EOE     The Pharmacist will be reporting to the Pharmacy Director and will be working closely with a team of tenured pharmacy technologists. Independently reviews and assesses medication orders to identify, prevent and resolve potential and actual drug-related problems. Contacts physicians, nurses and other professionals as necessary to facilitate care. Manages drug therapy protocols as part of physician-pharmacist-nurse team. Performs specialized order entry functions and maintains patient pharmaceutical care documentation within the clinical information system. Oversees daily drug distribution service activities. Responds to questions from physicians and nursing staff and patients regarding medicines and pharmaceuticals. Monitors individual patient drug therapy for medication safety and appropriateness. Monitors and updates charge system for drugs and pharmaceutical services Acts as a resource person for Medical Practitioners and hospital staff concerning information on medications. Documents drug therapy clinical interventions, medication errors, and adverse drug reaction events per pharmacy operational protocol. Supervises daily operation of Automated Drug Dispensing Devices. Promptly checks the verification order que. Orders medications for providers as needed QualificationsBe a graduate of a College of Pharmacy accredited by the American Council of Pharmaceutical Education Current pharmacist license for WA State 5 years of Pharmacy experience in applicable area
