Job Description

Job Summary

The Social Worker at Shriners Hospitals for Children provides comprehensive psychosocial assessment, intervention, and support to patients and families facing complex medical conditions and hospitalizations. This role focuses on empowering families, advocating for patient needs, and facilitating access to community resources to promote optimal patient well-being and successful reintegration.

Key Responsibilities

  • Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.

  • Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.  

  • Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.

  • Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve and share accountability for optimal outcomes.

  • Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.

  • Other duties as assigned.

Required Qualifications

  • Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers, required

  • Social Work license in state of practice

  • Demonstrated competency in Microsoft Office Suiteâ„¢

Preferred Qualifications

  • 3 years' experience

  • Experience in a Healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/family adherence, in a Pediatric setting

  • Current Case Management Certification (CMC, ACM or ANCC)

  • Bilingual in English/Spanish


Compensation is determined based on years of relevant experience and departmental equity.
