Care Coordinator - Non-Medical II

Fairview is looking for a driven Care Coordinator  to join our team at the Fairview Clinic in St.Paul, Minnesota! Clinic hours are Monday through Friday from 8:00 a.m. to 5:00 p.m. This position is 1.0 FTE (80 hours per pay period) every two weeks. 

Works within the Care Coordination team to provide patients with assistance in complex specialty scheduling, preventative care, and transitional care. Facilitates ongoing programs with community partners. Facilitates care as directed by physicians or RN's related to patient panel lists. Provides clerical and administrative support to staff and faculty, by managing their phone calls and calendars, completing correspondence, maintaining files, creating reports, and scheduling complex appointments.  Assists Care Coordinators and specialty providers with daily activities by managing clinic schedules that meet the needs of their patients, obtaining patient records, assisting with pre-visit planning and entering data into electronic medical records as directed. This position will work most closely with the RN Care Coordinator(s), and will be the subject matter expert regarding specific populations.

Responsibilities

• Completes the Care Coordination process for their panel patients.
• Liaison between clinic and community partners for shared initiatives, and long term collaborations.
• Facilitates complex health referrals.
• Promotes team environment within department, other site departments, and other clinics by acting as the Care Coordination subject matter expert.

Required Qualifications

• Basic health care insurance coverage knowledge.
• 2 years of Healthcare experience
• Use of EMR.

Preferred Qualifications:

• B.S./B.A. in Human Services or related field or
• 2-3 years experience in care coordination
