Concierge Home Care is hiring a Home Health Physical Therapist (PT). If you’re looking for more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity—this is the role for you.

Coverage Area: Tallahassee, FL / Leon, Franklin, or Taylor Counties
(Territories designed to minimize drive time with clustered scheduling)

Compensation & Earning Potential

• Salary+ model: $88,000 – $95,000+ for 24 productivity points
• Pay-per-visit at $70+ per visit + Medicare differential pay for all visits over 24 points
• Quarterly bonus opportunities 
• Mileage reimbursement + data plan stipend

Your Role as a Home Health Physical Therapist

• Evaluate patients and develop individualized treatment plans 
• Provide therapeutic exercises and mobility training 
• Educate patients and caregivers on safety and recovery 
• Collaborate with physicians and interdisciplinary teams 
• Supervise Physical Therapist Assistants (PTAs) 
• Complete documentation using streamlined technology
