JobDescription Summary

TheNurse Practitioner (NP) provides comprehensive, patient-centered clinical careto hospice patients and their families in collaboration with the hospiceinterdisciplinary team. The NP evaluates patient needs, develops and managesindividualized Plans of Care, provides clinical oversight, and supports thedelivery of high-quality hospice and palliative care services. The NP worksclosely with physicians, nurses, and other members of the interdisciplinaryteam to promote comfort, symptom management, quality of life, and continuity ofcare. Duties may evolve based on organizational and operational needs.

EssentialJob Functions/Responsibilities

• Conduct comprehensive patient assessments and evaluations to identify hospice and palliative care needs, including physical, psychosocial, and functional status.
  
• Provide ongoing clinical management of hospice patients, including assessment and treatment of symptoms, pain, and other conditions affecting patient comfort and quality of life.
  
• Develop, review, and update individualized patient Plans of Care in collaboration with the attending physician, hospice medical director, nurses, and interdisciplinary team members.
  
• Perform hospice visits, including routine, urgent, and follow-up visits, as clinically indicated and in accordance with patient and organizational needs.
  
• Collaborate with the hospice medical director, attending physicians, nurses, and interdisciplinary team members regarding patient status, changes in condition, treatment recommendations, and goals of care.
  
• Evaluate and manage medications and treatments related to symptom control, comfort, and overall patient care, including prescribing within the scope of licensure and organizational policies.
  
• Provide education and support to patients, families, and caregivers regarding disease progression, symptom management, medications, treatment plans, and end-of-life care.
  
• Participate in hospice admissions and evaluations as assigned, including determining and documenting the patient's clinical needs and appropriateness for hospice services in accordance with applicable requirements.
  
• Monitor changes in patient condition and communicate clinically significant findings and recommendations to the appropriate members of the interdisciplinary team.
  
• Participate in interdisciplinary team (IDT) meetings, care planning, case reviews, and other clinical coordination activities.
  
• Provide clinical documentation that is timely, accurate, complete, and compliant with Medicare Conditions of Participation (CoP), state regulations, and federal hospice requirements.
  
• Complete and maintain required clinical documentation, orders, certifications, re-certifications, and other hospice records within applicable regulatory and organizational timeframes.
  
• Support quality and regulatory compliance initiatives, including clinical audits, chart reviews, Quality Assessment and Performance Improvement (QAPI), and implementation of clinical best practices.
  
• Assist with education, consultation, and clinical support for nursing and other interdisciplinary staff as appropriate.
  
• Maintain current knowledge of hospice, palliative care, clinical standards, regulatory requirements, and evidence-based practices relevant to the role.
  
• Perform other duties as assigned.
  

PositionQualifications

• Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  
• Current and unrestricted Nurse Practitioner license in Oregon state.
  
• Current national board certification as a Nurse Practitioner through an appropriate recognized certifying body.
  
• Current DEA registration and prescriptive authority, as applicable.
  
• Minimum of two (2) years of clinical nursing or Nurse Practitioner experience preferred.
  
• Hospice, palliative care, home health, geriatrics, or other relevant clinical experience preferred.
  
• Demonstrated knowledge of hospice principles, symptom management, end-of-life care, and patient-centered care.
  
• Knowledge of Medicare Conditions of Participation and applicable state and federal hospice requirements preferred.
  
• Strong clinical judgment, critical thinking, communication, assessment, and care coordination skills.
  
• Ability to work collaboratively with patients, families, physicians, and interdisciplinary team members in a hospice environment.
  

WorkingConditions & Expectations

• Primarily office-based with regular visits to patient care environments as required.
  
• May require travel to patient homes, facilities, and other care settings.
  
• Must be available to respond to patient and clinical team needs outside of regular business hours as required.
  
• Works closely with patients, families, physicians, nurses, and interdisciplinary team members in a fast-paced and emotionally sensitive hospice care environment.
  
• May be exposed to patients with communicable illnesses and other healthcare-related environmental conditions.
  

PhysicalRequirements

• Prolonged or considerable walking or standing.
  
• Ability to lift, position, and assist patients as necessary.
  
• Ability to lift and carry supplies and medical equipment.
  
• Frequent reaching, stooping, bending, kneeling, and/or crouching.
  
• Ability to travel between patient care locations as required.
  
• Visual and auditory acuity sufficient to perform required clinical and patient-care functions.
