Lead CNA Job Description
Job Overview: The Lead CNA plays a critical leadership role in delivering high-quality, client-centered home care services. This position combines hands-on caregiving, staff training, and quality assurance oversight to ensure services meet regulatory standards and align with Thrive’s holistic care model.
Pay Rates:Â $20-21 an hour
 Responsibilities:
Direct Client Care & Coverage
- Provide hands-on care to clients.
- Cover open shifts as needed to ensure continuity of care.
- Travel throughout service areas to support client needs.Â
- Ensure client safety and maintain a safe home environment.Â
- Report any changes in the client’s condition promptly.Â
Quality Assurance & Compliance
- Develop, implement, and oversee quality control processesÂ
- Conduct caregiver performance evaluations and field assessmentsÂ
- Perform care plan audits to ensure compliance with regulationsÂ
- Ensure adherence to company policies and home care standardsÂ
- Assist with incident reporting and corrective actionsÂ
Training & Staff Development
- Train and onboard Personal Care Coaches (PCCs)Â
- Provide ongoing coaching and mentorship to caregiversÂ
- Educate staff on:Â
- Best practicesÂ
- Care standardsÂ
- Regulatory requirementsÂ
- Support continuous professional developmentÂ
Care Plan Oversight
- Review and evaluate individualized care plansÂ
- Collaborate with nurses and administrative staff to update care plansÂ
- Ensure services align with clients’ unique needs and preferencesÂ
Client Satisfaction & Relationship Management
- Monitor client satisfaction and service qualityÂ
- Address concerns and resolve issues proactivelyÂ
- Build strong relationships with clients and familiesÂ
- Foster a positive care environmentÂ
Documentation & Reporting
- Maintain accurate and timely documentation of:Â
- Client careÂ
- Caregiver performanceÂ
- Quality assurance activitiesÂ
- Ensure all records are compliant and audit-readyÂ
Administrative & Team Support
- Assist with scheduling assessments and onboarding processesÂ
- Support administrative tasks and team initiativesÂ
- Participate in interdisciplinary team meetingsÂ
- Collaborate across departments to improve service deliveryÂ
Continuous Improvement
- Identify gaps in care delivery and recommend solutionsÂ
- Implement initiatives to enhance quality and efficiencyÂ
- Support innovation in home care practicesÂ
Skills/Qualifications:
- Bachelor's degree in healthcare administration, nursing, or a related field OR a mix of education and experience.
- Certified Nursing Assistant certification (required).
- CPR and First Aid certification (required).
- Must have a valid Driver's License.
- Previous experience in a healthcare or home care setting.
- Knowledge of home care regulations and industry best practices.
- Excellent communication and interpersonal skills.
- Attention to detail and a commitment to continuous improvement.
- Familiarity with relevant technology and software for documentation and reporting.
- This position does require travel throughout our service regions.Â
 Technical Requirements:
- Must own a smartphone with data.
- Must own a personal computer to use for work purposes.
- Must own a personal vehicle with insurance that meets minimum state guidelines.
- Must be willing to travel to all our service regions.
- Must be willing to report to the office as needed.
