Summary


We are seeking a DFG Service Coordinator to join our dynamic Adult Social Care team for a 3-month contract. This role is pivotal in supporting the delivery of the Disabled Facilities Grant (DFG) Home Adaptations Service. As a customer-focused position, you will provide essential administrative and coordination support to help residents access home adaptations, enabling them to live safely and independently. You will serve as the primary contact for residents, manage DFG referrals, coordinate service activities, liaise with contractors and partner agencies, and ensure the efficient processing of applications.

Responsibilities


  • Manage and process Disabled Facilities Grant referrals using the case management system.
  • Serve as the first point of contact for residents, carers, contractors, and partner organizations.
  • Provide administrative support to the Home Adaptations Service.
  • Assist residents with DFG applications and related inquiries.
  • Conduct home visits as needed to support residents with administrative processes.
  • Maintain accurate records and update case management systems.
  • Coordinate customer surveys and KPI reporting.
  • Raise purchase orders, process invoices, and support payment processes.
  • Arrange meetings, take minutes, and manage correspondence.
  • Liaise with internal departments, contractors, and external agencies.
  • Respond to service requests, inquiries, and complaints professionally and promptly.
  • Support the effective delivery of a person-centered home adaptations service.

Requirements

Requirements:

  • Experience in an administrative or service coordination role.
  • Experience interacting directly with customers, residents, or service users.
  • Strong organizational and administrative skills.
  • Experience maintaining databases and computerised record systems.
  • Excellent communication and interpersonal skills.
  • Ability to manage competing priorities and meet deadlines.
  • Proficiency in Microsoft Office and IT systems.
  • Understanding of confidentiality and data protection requirements.
  • Ability to work effectively both independently and as part of a team.
  • GCSE English and Maths (or equivalent).
  • Enhanced DBS.

Desirable


  • Experience within Adult Social Care, Housing, DFG Services, or Local Government.
  • Experience processing referrals and managing case records.
  • Knowledge of home adaptations or Disabled Facilities Grant processes.
  • Experience working with contractors and external partners.
