Schedule:Â Full-Time, Monday-Friday, 8am-4:30pm
How you'll make an impact in this role
- Mentor Colleagues: Help junior case managers with complex cases and prioritizing their tasks
- Improve Processes: Review case management data to identify trends and enhance workflows
- Prevent Readmissions:​ Develop detailed discharge plans to lower 30-day readmission rates
- Ensure Continuity of Care: Support patients from admission through discharge to prevent care gaps
What minimum qualifications you'll need
Licensure / Certification / Registration:
- Licensed Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.
- Case Manager credentialed from the American Case Management Association (ACMA) preferred.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
- High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degreerequired. Master's degree preferred.
What additional requirements you'll need
- 5+ years of RN experience
- 3+ years of Case Management experience
- Hospital discharge planning experience preferred
