Job Title: LPN 

  

Department: Addiction Stabilization Center (ASC) 

  

Job Summary: This position is responsible for providing direct care to chemically dependent individuals, as well as direct supervision of primary care staff. 

  

Essential Job Functions  

  • Screen potential clients. 
  • Initiate and follow all medical and/or psychiatric emergency procedure in situations dictating need for such interventions. 
  • Obtain Vital Signs. 
  • Obtain, transcribe and administer medications per physician order and agency policy.  
  • Provide direct clinical nursing care per physician orders and in accordance with all clinical protocols. 
  • Monitor and report withdrawal process and medical condition of any clients under the influence of alcohol or other chemical substances. 
  • Arrange for the transfer of any client needing medical evaluation or higher level of care to local hospital as necessary. 
  • Correctly document or oversee related documentation including shift notes, opening notes, behavioral incidents, obtaining prescriptions and refills form, admission and discharge reports and all NYS OASAS forms.  

  

Minimum Job Qualifications  

  • Current and valid New York State LPN license. Maintains all required credentials. 
  • Must obtain CPR certification within 90 days of hire and maintain current certification throughout period of employment. 
  • Strong interpersonal communication skills.   
  • Intermediate knowledge of Microsoft Word, Excel and Outlook.

PHYSICAL JOB REQUIREMENTS

The tasks described signify many of the physical requirements of this position, based on a Time Weighted Average of an 8-hour work day.

  • Lifting up to 25 lbs. at waist level, up to 20 lbs. overhead
  • Twisting at the torso in stationary position, while seated
  • Clerical tasks, repetitive motion (of wrist and hand)
  • Use of stationary or portable machinery: photocopier, computer, printer, fax machine, scanner, postage meter, calculator, telephone
  • Bending legs to lift heavy objects up to 25 lbs. from floor level
  • Standing for extended period (up to 2-3 hours at a time)
  • Sitting for extended period (up to 5-6 hours at a time)
  • Walking up and down stairs
  • Typing on a computer and using a mouse (up to 2-3 hours at a time)
  • Exposure to heat and cold

  

Status/Hours: Part-Time/Per Diem  – 12 hour shifts  

Part Time: 6pm-10pm - 16-20 hours per week

  

Pay Rate: $25.00-28.00 per hour  

$3.00 shift differential for overnights 8pm-8am
