At MSHS, you’ll have the opportunity to make a meaningful impact in a community-focused setting while working alongside a collaborative team providing whole-person care—including primary care, behavioral health, dental, and psychiatry services.   Position Summary Under the supervision of the Medical Director, the Advanced Practice Provider delivers comprehensive, team-based primary care services to patients across the lifespan. This role emphasizes preventative care, chronic disease management, and integrated behavioral health collaboration.   Key Responsibilities Provide direct primary care services in an outpatient setting Diagnose and treat acute and chronic conditions Collaborate with medical, behavioral health, and psychiatric providers Maintain accurate and timely documentation in the electronic health record Participate in quality improvement and patient-centered initiatives Support care coordination and population health efforts Promote patient education and preventative care strategies     Our Mission Dedicated to improving the health of our community, one person at a time—through affordable medical, dental and behavioral health care attracts and inspires a caring, competent work force.
