Description

Status: PRN / As Needed 

Location: Milwaukee County & Surrounding Areas

Allegiant Hospice is a new, mission-driven hospice agency serving Milwaukee County. We are seeking a compassionate PRN Nurse Practitioner to join our growing interdisciplinary team. This role provides as-needed clinical support, patient visits, symptom management, and collaboration with our Medical Director and Director of Clinical Services.

Requirements

 

Key Responsibilities

  • Provide PRN visits for hospice patients in homes, facilities, or community settings
     
  • Support symptom management, medication adjustments, and patient/family education
     
  • Complete face-to-face (F2F) encounters as assigned
     
  • Document timely and accurately in the EMR
     
  • Collaborate with the hospice team (RN case managers, social workers, chaplains, aides)
     
  • Communicate changes in condition to the Medical Director and DCS
     
  • Participate in IDG meetings as needed
     

Qualifications

  • Current WI Nurse Practitioner license (APRN)
     
  • Active DEA or willingness to obtain
     
  • Hospice or palliative care experience preferred (not required)
     
  • Strong communication, critical thinking, and compassionate bedside presence
     
  • Ability to work independently and provide timely visit availability
