Behavioral Health Clinician

West Warwick • Integrated Primary Care

About the Role

Are you a clinician who believes behavioral health care works best when it’s woven into primary care — not referred out to a waitlist? Thundermist Health Center is looking for a Behavioral Health Clinician to join the integrated care team at our West Warwick location. You’ll provide trauma-informed, culturally responsive behavioral health services to adults as your primary focus, with the ability to see children and adolescents as needed. You’ll respond to warm hand-offs from medical providers, deliver individual, group, and family therapy, partner with psychiatric prescribers, and conduct screenings — all within a patient-centered medical home serving West Warwick and the surrounding Kent County communities.

What You’ll Do

Clinical Assessment & Treatment

  • Provide trauma-informed and culturally responsive behavioral health services to adult patients
  • Respond to warm hand-offs from medical providers for same-day behavioral health consultations, brief interventions, and triage
  • Conduct brief screenings to evaluate patient and family behavioral health needs
  • Deliver individual, group, and family therapy using evidence-based approaches (CBT, DBT, Motivational Interviewing, trauma-informed care)
  • Assess clinical emergencies including suicide risk and risk of harm to others; determine appropriate levels of care
  • Provide consultation to medical providers on behavioral health management and support integrated treatment plans
  • Partner with psychiatric prescribers (PMHNPs/psychiatrists) on patients requiring medication management alongside therapy
  • Provide behavioral health services to children and adolescents as needed within scope and caseload capacity

Collaboration & Community

  • Collaborate with providers, community agencies, and individuals in each patient’s network of care
  • Build and sustain relationships with Kent County community partners and adult service agencies
  • Participate in the Behavioral Health on-call schedule
  • Provide referrals for more intensive services where appropriate

Documentation & Compliance

  • Complete same-day documentation in the EHR (eClinicalWorks) including screenings, progress notes, treatment plans, and insurance documentation
  • Ensure compliance with HIPAA, 42 CFR Part 2 (SUD confidentiality), Rhode Island licensure scope of practice, and HRSA/FQHC requirements
  • Support HRSA UDS reporting for behavioral health measures (depression screening, SBIRT, tobacco cessation counseling)

What We’re Looking For

You’re a clinician who can shift from a warm hand-off to a structured therapy session to a crisis assessment in the same afternoon. You’re comfortable in a medical setting, value team-based care, and bring both clinical rigor and genuine compassion to your work.

  • Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or a related field
  • Rhode Island licensure as LCSW, LICSW, LMHC, LMHC-A, or LMFT (comparable out-of-state licenses considered with ability to obtain RI licensure)
  • Minimum 2 years of clinical experience with demonstrated experience working with diverse populations
  • Working knowledge of DSM-5-TR/ICD-10, psychotherapy methodologies, and evidence-based treatment planning
  • Demonstrated ability to assess clinical emergencies and determine appropriate levels of care

Preferred:

  • LICSW licensure in Rhode Island strongly preferred
  • Prior experience in an FQHC, community health center, or safety-net behavioral health setting
  • Experience with integrated behavioral health / PCBH model including warm hand-offs in primary care
  • Training in SBIRT, CBT, DBT, Motivational Interviewing, and trauma-informed care
  • Experience with perinatal mental health, MAT/SUD support, or co-occurring disorder treatment
  • Bilingual (Spanish/English or Portuguese/English) strongly preferred for the Kent County communities served

Schedule & Location

Full-time (40 hours/week), benefit-eligible, based on-site at Thundermist’s West Warwick location. Schedule includes mornings, afternoons, and evenings as required. Participation in the Behavioral Health on-call schedule is required. Occasional travel to off-site meetings and other Thundermist sites as needed.
