CHC Social Work Assessor

Location: Fully Remote (UK)
Contract: Permanent, Full-time (37.5 hrs) 

We’re looking for an experienced Social Worker CHC assessors to join a leading organisation delivering Continuing Healthcare (CHC) assessments.

About the Role

Working within the National Framework for NHS Continuing Healthcare (CHC) and the Care Act 2014, you will:

  • Chair Multi-Disciplinary Team (MDT) meetings held online (MS Teams or Zoom) with patients, families, and care providers.

  • Complete CHC assessments and reviews using the Decision Support Tool (DST), providing clear eligibility recommendations.

  • Create person-centred care and support plans.

  • Liaise with local authority representatives and other professionals to gather evidence and ensure robust decision-making.

  • Work to agreed Key Performance Indicators (KPIs) for quality, productivity, and service delivery.

This is a fully remote role, giving you the flexibility to balance professional and personal commitments while still making a tangible difference to people’s lives.

What We’re Looking For

We welcome applications from Registered Social Worker backgrounds with at least 12 months experience and:

  • Previous experience completing CHC assessments and reviews within the NHS, Social Care or similar settings.

  • Strong understanding of the CHC Framework 2022, Care Act 2014, and Mental Capacity Act 2005.

  • Excellent written skills to produce clear, person-centred, evidence-based reports.

  • Confident communication and empathy when dealing with sensitive, often complex health and social care matters.

  • Ability to plan and organise workload, meet KPIs, and work collaboratively with other professionals.
