This role is full time based at the Hospital and you will benefit from working in a professional and passionate environment. As a valued member of the team you will be responsible for social care assessments, assessing the needs of patients and their carers, arranging appropriate care packages and services, liaising with other professionals and agencies and ensuring a safe and timely discharge. You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be SWE registered. Additionally, you must have at least two years post qualifying experience and be able to provide five years of references.
