Social Worker – Adults Locality Team

Main Responsibilities
  • Deliver high-quality social work services to adults and families, focusing particularly on complex cases.

  • Conduct comprehensive needs assessments and create tailored care and support packages.

  • Balance risk and autonomy to promote safe and independent living for service users.

  • Provide expert advice and support to colleagues, students, and other professionals.

  • Promote independence and informed decision-making through collaboration with individuals, families, and communities.

  • Design and commission cost-effective care solutions.

  • Engage in preventative interventions using tools such as reablement and telehealth.

  • Maintain accurate and timely records on both electronic and paper systems.

  • Present cases to the Resource Allocation Panel and contribute to legal proceedings where necessary.

  • Build positive working relationships with service users, carers, and partner agencies to gather information and resolve conflict.

  • Stay up to date with current legislation, policy, and best practices within adult social care.

Essential Skills and Knowledge
  • In-depth understanding of the Making Safeguarding Personal approach.

  • Solid working knowledge of the Mental Capacity Act 2005, Human Rights Act 1998, and Care Act 2014.

  • Strong professional ethics and values focused on empowering individuals and achieving positive outcomes.

  • Ability to work preventatively and collaboratively with families, carers, and the wider community.

  • Experience in creating efficient, value-for-money care plans and liaising effectively with multidisciplinary teams.
