What will your responsibilities be?

  • Conduct comprehensive assessments for individuals with complex physical, cognitive, or social needs
  • Recommend and implement appropriate aids and adaptations to promote independence and safety
  • Collaborate with service users, carers, and multidisciplinary professionals to develop tailored, person-centred support plans
  • Deliver high-quality care in a community setting, ensuring dignity, respect, and empowerment at every step

Qualifications & Experience:

  • Qualified Occupational Therapist with current HCPC registration
  • Proven experience in community-based work, particularly with aids and adaptations
  • Strong understanding of complex health and social care conditions
  • Excellent communication, interpersonal, and problem-solving skills
