Care Coordinator (Social Worker / Occupational Therapist - Band 6) Location: Appleby Health Centre, 63 Appleby Road, London, E16 1LQ Service: Newham Early Intervention in Psychosis (EIP) Team Contract Type: Locum - Full Time (preferred) Start Date: 26/05/2025 Duration: 3 Months (initially) Working Pattern: On-site with a maximum of 1 day WFH per week Pay Rate: Band 6 - Outer London - £30 per hour umbrella equivalent Job Summary: The Newham Early Intervention in Psychosis Team is seeking a skilled and motivated Care Coordinator (Band 6) with a background in either Occupational Therapy or Social Work to join their specialist community service. This role is focused on supporting individuals aged 18-65 who are recovering from their first episode of psychosis. Key Responsibilities: Act as the named Care Coordinator for a caseload of up to 21 individuals Deliver person-centred support using a biopsychosocial approach, in line with Early Intervention in Psychosis (EIP) principles Work collaboratively with service users, carers, and the wider multi-disciplinary team to create and implement effective care plans Provide regular mental state assessments, risk assessments, and ongoing reviews Monitor progress and recovery, adjusting care planning as appropriate Ensure accurate documentation and compliance with Trust policies and professional standards Engage in CPA reviews, safeguarding procedures, and discharge planning as needed Service Overview: Newham EIP is a forward-thinking and dynamic team supporting people during a crucial period in their mental health journey. The team provides intensive, time-limited intervention with a strong focus on recovery, early engagement, and reducing relapse. Requirements: Qualified Social Worker or Occupational Therapist, registered with Social Work England or HCPC/HCPC successor (HCPC-R) Previous experience working within a community mental health team or Early Intervention in Psychosis setting Strong knowledge of psychosis, recovery models, and relevant legislation (e.g., Mental Health Act, Care Act) Ability to manage a diverse caseload and deliver high-quality care coordination Excellent interpersonal, communication, and documentation skills Additional Information: Full-time preferred, part-time may be considered One day working from home per week (maximum) This is an exciting opportunity to work within a progressive and supportive team delivering recovery-focused care to individuals during a critical stage in their mental health journey.
