Job Overview:
As a Care Coordinator/Deputy Care Manager, you will work alongside the Care Manager to oversee the delivery of person-centred care services for our clients and develop our client base. You will coordinate needs assessment and care plans, build a new service from the ground up and manage a team of carers, and ensure the highest standards of care are consistently met. Key Responsibilities:

Assist the Care Manager in the day-to-day management of care services.

Develop and review individual care plans, ensuring they reflect the needs and preferences of clients.

Coordinate and monitor the delivery of care services, ensuring they meet regulatory and quality standards.

Supervise and support a team of care staff, providing guidance, training, and performance management.

Conduct client assessments and ensure appropriate care is provided in line with personalised care plans.

Act as a point of contact for families, external agencies, and healthcare professionals.

Ensure effective communication within the team and with other relevant parties.

Maintain accurate and up-to-date client records and documentation in line with company policies and procedures.

Promote a positive and supportive environment for clients and staff.
Skills and Experience:

Previous experience in a care coordination or deputy care management role, or a similar field.

Strong understanding of person-centred care principles and regulatory standards.

Excellent communication skills, both written and verbal.

Experience in managing and supporting a team of carers.

Good organisational and time-management skills with the ability to prioritise tasks effectively.

A compassionate and empathetic approach to working with clients.

NVQ Level 3 in Health and Social Care (or equivalent) is desirable; Level 5 is a bonus.
