What You'll Be Doing:

Join our client's friendly and forward-thinking GP practice as a Prescribing Pharmacist and become a key player in delivering exceptional patient care within the community. In this role, you will:

  • Perform comprehensive patient assessments

  • Create and implement personalised care plans

  • Deliver evidence-based treatments to optimise patient outcomes

  • Collaborate with multidisciplinary teams to provide holistic care

  • Maintain accurate patient records

  • Prescribe medications independently in accordance with clinical guidelines

  • Conduct medication reviews and optimise patient treatment plans

  • Provide expert advice on prescribing and medicines management within primary care

  • Support patients with long-term conditions through appropriate medication management

Requirements & Qualifications:

To be successful in this role, you will need:

  • A current and valid GPhC registration

  • Experience working in primary care / GP practices

  • Independent Prescriber qualification (essential)

  • Strong communication and interpersonal skills

  • The ability to work independently and within a multidisciplinary team

  • A commitment to delivering high-quality patient care

Working Hours & Contract Duration:

This is a full-time, on-site position working Monday to Friday (9:00 - 17:30, 37.5 hours per week). The contract is ongoing, with high potential for extension depending on performance and service needs.
