What will your responsibilities be?

In this role, you will support adults and young people with learning disabilities by completing Care Act Assessments and reviews. You will work collaboratively within a multi-disciplinary team, including Adult Social Care and NHS Health staff. Managing safeguarding cases, assessing risk and coordinating care management effectively are key responsibilities. You will engage with service users, families and partner agencies to promote person-centred support. Demonstrating strong communication, organisational skills and a commitment to improving outcomes for individuals with learning disabilities is essential. Your efforts will contribute to enhancing the quality of life for those you support.

Qualifications & Experience:

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level and have at least 2 years of post-qualified experience.
