Job Summary
The Advanced Clinical Practitioner (ACP) will work within a primary care hub, delivering expert clinical assessment, diagnosis, treatment, and ongoing management of patients. The role involves a mix of clinical care, leadership, education, and service development, supporting the multidisciplinary team and improving patient outcomes.
Main Duties
- Conduct comprehensive assessments including history-taking, physical examinations, and risk factor analysis.
- Diagnose, prescribe, and implement evidence-based treatments.
- Plan and manage complete episodes of care, ensuring continuity and safe patient management.
- Support patients and families in understanding and managing conditions, including sensitive/difficult discussions.
- Refer, delegate, and collaborate with MDT colleagues.
- Contribute to clinical leadership, staff mentorship, training, research, and service development.

About the Surgery
A collaborative GP Surgery based in Braintree, serving a population of approximately 12,000 patients.
The multidisciplinary team includes:
ANPs, PAs, Nursing Associates, Paramedics, FCPs, Pharmacists, Pharmacy Technicians, Care Coordinators, and Social Prescribers.
Person Specification
Essential Skills & Abilities
- Strong theoretical & practical skills in patient management
- Leadership & organisational skills
- Excellent written & verbal communication
- Ability to problem-solve & work independently
- Experience supervising, teaching, or mentoring staff
- Must have GP surgery experience
Essential Experience & Qualifications
- Registered with NMC / GPhC / HCPC
- MSc in Advanced Practice (or equivalent / Centre of Advancing Practice directory registered)
- Independent Prescribing qualification - Paramedics must be able to prescribe
Desirable
- Experience in audit, research, or service development
- Involvement in change management
- Ability to write reports and clinical protocols
