As a Case Coordinator, you will play a vital role in supporting individuals experiencing or at risk of homelessness to sustain their accommodation and build more independent lives. You'll work directly with clients to ensure their accommodation is safe and well-maintained, while also helping them manage personal finances, including benefit applications and budgeting support.
You'll be responsible for maintaining accurate, up-to-date records of all client interactions and will support the Service Manager and Regional Manager with data reporting to partners and funders as needed.
Key Responsibilities
Track and manage referrals into the service, as well as outcomes for clients exiting the service, using our internal case management system and the Bristol Housing Support Register (HSR).
Complete needs and risk assessments, leading to the delivery of personalised support plans tailored to each client's circumstances.
Ensure new clients understand their rights, responsibilities, and the terms of their licence agreements, and that a valid Housing Benefit claim is in place.
Deliver high-quality, person-centred support using a strength-based and trauma-informed approach.
Maintain a high standard of health and safety in client accommodation.
What We're Looking For
A valid UK driving licence and access to your own vehicle (business insurance required).
An understanding of the root causes of homelessness, offending, and social exclusion, and an appreciation of effective, client-focused approaches to addressing these issues.
Empathy, resilience, and a strong understanding of the challenges faced by people with complex needs and challenging behaviours.
Experience working with vulnerable or marginalised groups in a supportive, person-led role.
- On-call Requirement: 1 in every 4 weeks (105/week)
