Sonic Summary

The job involves arranging and monitoring services according to individual care plans and providing advice and practical help to service users.
Candidates must have experience working with children and families in a direct contact environment, ideally with a minimum of 2 years of relevant experience.
The role requires good communication, organizational, and administrative skills, as well as the ability to work both independently and as part of a team.
The position is mostly office-based but may require occasional visits to service users as needed.

Responsibilities

To arrange and monitor appropriate services in line with individual care plans.
To provide advice and practical help.
To liaise with voluntary and statutory agencies.
To complete annual care plan reviews and or reassessments.
To participate in team meetings and duty rotas.
To undertake these duties in line with Council Policies and Procedures.
To undertake training and self-development and to contribute to this process in others.

Requirements

Experience of working with children and families through direct contact in a working environment.
Ability to work independently and as part of a team.
Ability to make relationships with se5rvice users and carers.
Good communicative, organisational and administrative skills.
Mostly office based but could be called on to visit service users as needed.
