- Support timely and safe hospital discharges through integrated working.
- Carry out assessments and interventions in acute hospitals, community settings, and service users’ homes.
- Apply strength-based, personalised approaches to improve outcomes.
- Work within the framework of the Care Act, Mental Capacity Act, and Mental Health Act.
- Collaborate with colleagues and partners to deliver effective, resource-conscious care.
- Promote and uphold safeguarding policies and the welfare of vulnerable adults at all times.
- Experience in adult social care, ideally in hospital or discharge settings.
- Strong knowledge of relevant legislation.
- The ability to work in a fast-paced, dynamic environment while maintaining professional standards.
