Care Coordinator

A growing home care service in Darlington is seeking an experienced and organised Care Coordinator to support the expansion of its domiciliary care provision.

About the Role:

As a Care Coordinator, you will play a key role in managing care packages, maintaining compliance, liaising with professionals, and ensuring the delivery of high-quality, person-centred care. You'll work closely with clients and care staff to ensure the right support is provided at the right time.

Key Responsibilities for the Care Coordinator:

Coordinate care services and ensure timely allocation of staff

Liaise with social workers and other professionals to support service users

Complete person-centred risk assessments and care plans

Ensure all care plans are reviewed annually or when circumstances change

Match care staff to clients based on needs and compatibility

Investigate and record any complaints, incidents or accidents in line with policy

Visit clients when necessary to assess service delivery

Maintain accurate and up-to-date records

Participate in the out-of-hours on-call rota (shared responsibility)

Requirements for the Care Coordinator:

Previous experience as a Care Coordinator in domiciliary or home care

Excellent communication skills - verbal, written and interpersonal

Ability to develop and maintain professional relationships

High attention to detail and strong organisational skills

Proficient in Microsoft Office (Word, Outlook, Excel) and internal databases

Understanding of confidentiality and safeguarding

Strong customer service approach and a caring, can-do attitude

Flexible and reliable, with the ability to work in a fast-paced environment
