Job Title: Hospital Discharge Social WorkerHourly Rate: £26.26Location: [Hospital Location in Darlington]Job Summary:We are seeking a highly motivated and experienced Social Worker to join our dedicated Hospital Discharge Team in Darlington. You will play a pivotal role in facilitating timely and safe discharges for adult patients, working collaboratively with a multi-disciplinary team to ensure patients receive the right support and care to transition back to their homes or alternative care settings.Key Responsibilities:* Conduct comprehensive assessments of patient's needs, including physical, emotional, and social factors.* Develop and implement person-centred discharge plans that meet individual needs.* Liaise effectively with medical staff, nurses, occupational therapists, and other health care professionals.* Work collaboratively with community services, including social care teams, housing support, and other relevant agencies.* Advocate for patient's rights and ensure their voices are heard.* Facilitate timely and safe hospital discharges, minimising delays and risks.* Support patients and their families in their transition back to the community.* Maintain accurate and up-to-date case records.* Contribute to the ongoing development and improvement of the team's services.* Comply with all relevant legislation and professional guidelines.Essential Criteria:* A recognised Social Work qualification* Current registration with the Health and Care Professions Council* Significant post-qualifying experience in adult social work.* Experience working within a hospital or health-care setting is highly desirable.* Excellent communication, assessment, and care planning skills.* Ability to work independently and as part of a multi-disciplinary team.* Full UK driving licence and access to a vehicle.Desirable Criteria:* Experience working with specific patient populations (e.g., older adults, people with mental health conditions).* Knowledge of relevant legislation and policy frameworks (e.g., Mental Capacity Act).* Experience with case management software.Benefits:* Competitive hourly rate of £26.26.Apply
