Are you a driven and compassionate Social Worker passionate about supporting adults with learning disabilities through significant life transitions? We're seeking a dedicated professional to join our Transitions Team in Doncaster.
About the Role:
- Work directly with adults with learning disabilities, supporting them to navigate complex transitions such as moving to more independent living, changing support providers, or accessing new services.
- Develop and implement person-centered transition plans, ensuring seamless and positive outcomes.
- Collaborate closely with families, care providers, and other professionals to coordinate support and advocate for individual needs.
- Conduct thorough assessments and provide ongoing support and guidance to individuals during their transition journey.
- Contribute to a supportive and dynamic team committed to improving the lives of adults with learning disabilities.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a Social Worker.
- Experience working with adults with learning disabilities, ideally within a transition-focused setting.
- Excellent communication, problem-solving, and case management skills.
- A proactive and solution-oriented approach to supporting individuals through change.
- Commitment to person-centered care and ethical practice.
