About the Role:

Join the Integrated Discharge Team (IDT) in Doncaster as a Social Worker, where you will facilitate smooth transitions from hospital to home, ensuring comprehensive care and support for patients. If you are passionate about making a difference, apply now!

What will your responsibilities be?

In this role, you will work within the Discharge Assessment and Review Team (DART) at Doncaster Royal Infirmary, supporting discharges from hospital and the emergency department for adults aged 18 and above. You will carry out strength-based assessments under the Care Act 2014 and Mental Capacity Act 2005, focusing on safe discharge planning with a Home First ethos. Collaborating with health professionals, you will identify and arrange appropriate support, whether returning home, accessing rehabilitation, or further assessment in a bed-based setting. You will operate across various settings to facilitate effective discharge planning, ensuring timely, person-centred outcomes aligned with hospital discharge policy and best practice.
