About the Role:
Our client in Wiltshire is looking for a passionate, motivated, and experienced Mental Health Social Worker to join their team. This role involves working within the community mental health setting to carry out social care assessments, develop care plans, and support patients during their treatment and recovery process. This is a small, well-established, dedicated social care service for people with severe and enduring mental health conditions.
What will your responsibilities be?
In this role, you will conduct thorough social care assessments to identify the needs of patients within the community. You will develop and implement effective care plans tailored to individual needs and collaborate with medical, nursing, therapy, and mental health teams to ensure smooth and timely treatment plans. Managing a caseload, you will provide support with housing, access to benefits, and practical support at home for patients receiving secondary mental health services. Additionally, you will work with specialist teams for early intervention in psychosis, assertive outreach, dual diagnosis, and psychological therapies. You will operate in a hybrid model, working both from home and community sites as required, and undertake other assessments such as Mental Capacity Assessments (MCA), Safeguarding enquiries, and Continuing Health Care (CHC) checklists as needed. Finally, you will assess, coordinate, and arrange care for service users with mental health problems, including dementia, both in the community and in the hospital.

What we are looking for in a Social Worker:
- SWE registration
- Eligible to work in the UK
- Hold a full UK license
- Post-qualified experience
- Degree level or equivalent in Social Work
