Social Worker - Next Steps Team | Durham County Council

Help young people take their next steps toward independence.

Durham County Council is seeking an experienced Social Worker to join our newly established Next Steps Team, supporting 16- and 17-year-olds as they prepare for adulthood. You'll work alongside young people to create Pathway Plans, set aspirational goals, and ensure a smooth transition to care leaver services.

Key Details:

  • Location: Pity Me, Abbeywoods Business Park, Units 4, DH1 5TG
  • Rate: 28.42 per hour
  • Contract: Temporary
  • Start Date: ASAP

What You'll Do:

  • Manage a caseload of young people aged 16-17
  • Collaborate with care leavers and children in care services
  • Work closely with wraparound support including Staying Close, Peer Mentoring, and emotional wellbeing services
  • Empower young people to achieve their goals and independence

About You:

  • Qualified Social Worker with Social Work England registration
  • Experience in working with adolescents and care planning
  • Strong communication and relationship-building skills
  • Passion for supporting young people through transition
