- Hold a caseload of up to 25 young people
- Provide tailored, one-to-one support for care leavers transitioning into adulthood and independent living
- Work collaboratively with services including education, training, employment, housing, and health to ensure holistic support
- Carry out office-based work, community visits, and attend meetings both locally and out-of-borough as needed
- Empower young adults to develop and work towards their care and health plans, fostering confidence, resilience, and independence
- Ensure all support is delivered in line with local procedures, policies, and relevant legislation (e.g. the Children (Leaving Care) Act 2000)
- Significant experience of working with vulnerable young people with a range of complex problems
- Knowledge of the legislative framework around Looked After Children and Care Leavers
- Demonstrable success in partnership working for the benefit of young people
- A specialist consultant within an experience and dedicated recruitment team
- Excellent rates of pay
- Payroll services twice a week
