The Role:

Our client is seeking a dedicated Rough Sleeper Prevention Worker to support individuals experiencing rough sleeping in Exeter. This role involves engaging with people both on the streets and within temporary accommodation, including emergency sleeping pods. You will deliver high-quality, person-centred, assertive outreach as part of a collaborative team, working with new, repeat, and long-term rough sleepers across the Exeter City Council area.

Please note: This role requires flexibility, including working weekends and unsocial hours, and the ability to walk several miles in all weather conditions.

Key Responsibilities:

  • Conduct assertive outreach and hotspot visits across the local area
  • Manage referrals and support individuals accessing emergency accommodation
  • Complete needs assessments, support plans, and risk assessments to help individuals access appropriate services
  • Empower a caseload of clients by identifying their strengths and goals, and supporting their development and wellbeing
  • Maintain accurate and up-to-date client records using the internal case management system
  • Undertake other duties in line with the role as required

Requirements:

  • A valid UK driving licence and access to a vehicle (business insurance required)
  • Experience working with vulnerable adults and/or individuals with complex needs and challenging behaviours
  • Strong interpersonal skills with the ability to build effective relationships with clients and partner services
