Qualified Social Worker, Mental Health TeamPay rate £38.57 per hour Contract roleVitalis are recruiting for an experienced Social Worker to work in a Mental Health Team in Devon.We are recruiting for a qualified, experienced social worker to join a community mental health social care team, based in the Mendip area of somerset. You will carry out assessments in line with legislation including the Care Act and Mental Capacity Act and which promote recovery and independence and apply for funded care where applicable. You will need to maintain accurate, succinct records using electronic record systems and clearly document the rationale for decision-making. The work we do is underpinned by a set of guidelines which include:
  • Focusing on ‘connecting people to people’ rather than ‘people to services.
  • Starting with the assets and strengths of people, their families and their communities and thinking about services last.
  • Gaining and maintaining knowledge of the families, neighbourhoods and communities you work with in order for conversations to be effective.
  • Converting conversations into outcomes and ask the question ‘how will we know when you have achieved this? A current driving licence is essential. We offer hybrid working, this will include time spent office based, home based and time visiting people living in the community.
