Job Purpose:

To provide high-quality, person-centred support to individuals who are homeless or at risk of homelessness, helping them to sustain tenancies, build independence, and move on to longer-term housing solutions. You will work collaboratively with referral agencies and support networks to deliver tailored support and housing-related services.



Key Responsibilities:

  • Engage with actual and potential referral agencies (e.g. local authority gateway services, statutory/voluntary agencies) to generate and manage referrals.
  • Develop and deliver personalised support and safety plans, referring to specialist agencies where appropriate.
  • Record, monitor, and regularly review support plans, ensuring service user contact and outcomes are tracked in line with contractual requirements.
  • Ensure properties are properly equipped, maintained, furnished, and cleaned to a high standard.
  • Issue and manage tenancy or licence agreements, ensuring service users understand and comply with expectations.
  • Take appropriate action in cases of non-compliance or disengagement, including partnership working or enforcement as needed.
  • Empower service users to set and achieve realistic goals through regular engagement and motivational support.
  • Facilitate service user involvement in decisions that affect them, promoting independence and responsibility.
  • Identify move-on options early and support clients to develop sustainable housing pathways.
  • Support service users in developing life skills and completing domestic tasks, offering practical help where necessary.
