Eden Brown Synergy is recruiting an experienced Qualified Social Worker to join the Hospital Discharge and Post-Assessment Discharge Team in Calderdale.
Role Overview:
Supporting safe, timely hospital discharges
Completing post-discharge assessments
Working across Calderdale Royal Hospital, Huddersfield Royal Infirmary and the local community
Hybrid working available
Experience in hospital discharge is essential
